Dr. Michael and Marlene Nissenblatt Scholarship

Purpose: First or second year post-graduate students of Western medicine, traditional Chinese medicine, acupuncture and/or herbology who embrace and bring the human touch and human spirit to health care may apply.

Award amount: $2000

Candidate criteria:

1.    Graduate from a New Jersey high school.

2.   United States citizen.

3.   Attainment of a bachelor’s degree.

4.   Admission to a Western medicine, traditional Chinese medicine, acupuncture or herbology program at a certified school located in the United States. Medical students must be matriculating in any non-research clinical medical field.

Application deadline:  Postmarked no later than August 1, 2008

Submittals: Six copies of the application packet mailed to the Chamber Charitable Foundation with “Dr. Michael and Marlene Nissenblatt Scholarship” on the envelope:

1.    A completed application form, available from the Chamber website.

2.   The name, town and state of the high school from which s/he graduated.

3.   Declare other support they have applied for or received.

4.   A transcript for the most recent level of study.

5.   A letter confirming acceptance to the school and documentation of payment to the program of study.

6.   Three letters of reference, each with the name, title, institution or organizational affiliation and telephone number of the reference.

a.   References who send the letter by surface mail must sign the letter and mail it in a sealed envelope with “Dr. Michael and Marlene Nissenblatt Scholarship” on the outside.

            References may email the letter directly to the Chamber Charitable Foundation with “Dr. Michael and Marlene Nissenblatt Scholarship” as the subject.
